Supporting Our Veterans with Every Visit

The Hospital Program is a vital part of our Department's mission to care for veterans, their families, and those in need. Every hospital visit, phone call, card, or small act of kindness matters—and we want to make sure it's counted. Please use the form below to submit your monthly hospital report.

How to Report

Please fill out the form completely. Reports should be submitted by the 10th of each month to ensure they are included in Department totals.

You may report:

  • In-person visits to hospitals, VA clinics, or care facilities
  • Cards or packages sent to hospitalized veterans
  • Phone calls made to check on the health or wellbeing of members or their families
  • Volunteer time supporting healthcare-related events
Note: If you visited multiple individuals in one location, please count each individual visit. For example, 3 veterans visited in a VA hospital = 3 visits.
